Dear Valued La Belle Vie Clients,
We are excited to announce that La Belle Vie Cosmetic Surgery Center will be re-opening our doors and returning to normal business hours as of Monday, May 18, 2020. We have been working tirelessly to ensure that, upon opening, we place patient safety and health above all other priorities. In order to do so, we will be implementing some changes that allow us to adhere to the current recommendations for social distancing and sanitation.
Before arriving at our LBV office, we need you to complete all your intake information at home and email it to us BEFORE your appointment.
- All visitors must complete our COVID-19 Screening Questionnaire (if any of your answers are yes, please reschedule your appointment).
- New patients must complete our: Registration information, Medical history information, Insurance information (including a front and back copy of your insurance card).
Please keep your mobile phone with you at all times since communication is important to keep you on time. And remember that you MUST wear your mask.
Arrival at our LBV office will be different until COVID-19 dissipates. Here’s the current procedures for entering LBV now:
- Call the office to inform them of your arrival.
- Confirm receipt of your intake forms and your negative COVID-19 Questionnaire.
- Stay in your car until notified to enter.
- When notified, come to the front door wearing your mask where your temperature will be measured.
- Come alone if possible, however, one (1) other person will be allowed to enter with you if they are needed.
- Please DO NOT bring children as they are an unnecessary risk.
- You will be escorted to the exam room where you will remain until the end of your visit. After your visit, you will take all materials and immediately exit the office.
